New Patient Information Patients that are making their very first visit to the Alamogordo Eye Clinic will be required to fill out the New Patient Information Form. You will also be required to read the Notice of Privacy Practices and fill out the Acknowledgement of Receipt of Notice of Privacy Practices Form. You can obtain these forms now by clicking on the links below. It will save you the time and inconvenience at the Eye Clinic if these forms are filled out prior to your first visit.
Your eyes may be dilated during your visit to the Eye Clinic. Dilation causes impaired vision so we advise our patients to prepare for this by bringing along a pair of sunglasses and to make arrangements for someone else to drive you home.
